MedPlex Ambulatory Care Center Detailed insurance information will be obtained at the time an appointment is scheduled and then verified during the pre-registration process. The patient must bring two forms of identification (preferably a driver's license and social security card) as well as insurance cards to each clinic visit. The patient will be contacted two to three days prior to the scheduled appointment to review insurance and personal information and will also receive a reminder telephone call within three days of the scheduled appointment.
