Pharmacy Managed Direct Patient Care Clinics

Anticoagulation Clinic
This clinic provides anticoagulation management for approximately 300 patients receiving anticoagulation therapy. Residents are responsible for patient interviews, physical assessment, therapeutic assessment, and decision-making.

Diabetes Education Classes
Patients are referred from Internal Medicine, High Risk Obstetrics, and various area community clinics for diabetes education. Patients attend two classes that are five hours in length. This program is certified by the American Diabetes Association. The ambulatory clinical specialists, residents and a nutritionist teach the classes. Residents are responsible for educational assessments, patient education, and setting goals for behavior modification.

Diabetes, Hypertension, and Hyperlipidemia Clinic
Patients are referred to this clinic from Internal Medicine for aggressive treatment of diabetes, hypertension and hyperlipidemia. Residents are responsible for patient interviews, physical assessment, therapeutic assessment, and decision-making.

HIV-AIDS Service
This service provides patient education, compliance counseling, and medication monitoring for outpatients in the state's largest HIV-AIDS Clinic. This service also monitors patients when they are hospitalized. Residents are responsible for all aspects of this service.

Inpatient Anticoagulation Service
The resident participates in this pharmacy-managed service that provides consultations for the management of anticoagulation therapy for all inpatients. Residents provide written consultations in the medical record on all patients receiving oral anticoagulation and monitor low-molecular-weight heparin therapy in special populations. Pharmacists coordinate the Lovenox Early Anticoagulation Program (LEAP). Pharmacists may be consulted to collaboratively manage anticoagulation therapy. The Anticoagulation Service is provided 365 days a year.

Outpatient Anticoagulation Service
This service provides management of anticoagulation therapy for home health patients referred by Orthopedics, Surgery and Internal Medicine.

Outpatient Pharmacokinetic Service
This service provides pharmacokinetic monitoring and management of home health patients receiving selected antibiotics. Patients are referred from Orthopedics, Surgery and Internal Medicine.
